You can now self refer yourself directly to several departments within the Southern Health and Social Care Trust without having to contact your GP.

Musculoskeletal Physiotherapy Self Referral Form

If you are registered with a GP in the Southern Trust, over 16 and have a back or neck problem, a joint/muscular pain or have a recent injury such as a sprain, then you can self refer to Physiotherapy by completing the online form here.

Maternity Self Referral Form

If you have a positive pregnancy test result, you can self refer for maternity care through your midwife by clicking here.

Stop Smoking Services

Due to COVID-19, all face to face appointments with the smoking cessation officer have been suspended. You can still avail of telephone support. Click here to complete the online referral form.

Better Days Pain Support Programme

This pain support programme will be run virtually via Zoom. It aims to help people with chronic pain to reduce their reliance on medication through better understanding of the limited role of medicines, and to learn about other ways to help manage their pain and improve their quality of life.

Bookings can be made online at https://www.hlcalliance.org/page/pain-support-book-now. There are a total of 12 places in the Newry class which starts on the 20th May 2021 at 11am and is weekly for 8 weeks with each session lasting for approximately 2 hours.
